8442 - REPORTING ACCIDENTS
Timely reporting of all school-related accidents is critical to the coordination of insurance coverage protection. Internal forms have been designed to facilitate these reporting requirements. In addition to the immediate filing of these forms, two (2) considerations must be noted:

A. |
In no case, should any arrangements or detailed discussions be conducted with parties alleging injury. Refer them to Human Resources, if necessary. |

B. |
In no case should you make direct contact with any insurance company. All communications, written and verbal, will be coordinated by Human Resources. |
Student Accident
All student visits to the clinic are recorded in the clinic log (date, time, nature of visit, and action taken).
When deemed appropriate a student accident report (Form 5340 F1) must be completed by the teacher, bus driver, or other school personnel.
Student Accident Forms are to be completed for any injury that results in:

A. |
subsequent absence; |
|
|
B. |
student to student injury; injury inflicted by another student (i.e. bite, fight); |
|
|
C. |
head injury resulting in swelling (edema), dizziness, loss of consciousness, or nausea; |
|
|
D. |
student dismissal from school; |
|
|
E. |
squad or parent/guardian being summoned; |
|
|
F. |
referral for medical care. |
When completing the required documentation, note, in particular, the following:

A. |
give full details of accident |
|
|
B. |
be very specific in describing apparent injury |
|
|
C. |
obtain name, address, and telephone number information for any witnesses and/or others present at time of accident |
This form should be previewed by the school nurse and principal and forwarded to the Treasurer.
Employee and or Visitor Accident
In the event of an employee and or visitor accident, however slight, an Employee Accident Report (Form 8442 F2) must be submitted to the Coordinator of Human Resources within twenty-four (24) hours, if possible.
School Vehicle Accidents
Motor Vehicle Accident Report must be filed with the Director of Business Operations within twenty-four (24) hours of the accident. The responsible administrators should ensure that a complete description of the accident is contained in the report; indicating witnesses, unusual circumstances, etc. In all cases, evidence of filing a police report must be furnished with the accident report.
